Propane explosion: Equipment failure with maybe human error

Tavares/Orlando, Fla. - Eight people were hurt in a series of explosions at a propane plant in Florida Monday in a blast producing a fireball that lit up the sky northwest of Orlando

Federal officials have launched an investigation into the incident at the Blue Rhino Propane Plant.

Eight workers were injured, four critically.

Chief Richard Keith of the Tavares Fire Department said, "We don't think that there was any act of sabotage or anything like that. We honestly think it was probably an equipment failure with a combination of maybe human error from one of the staff."

One of those injured was a worker hit by a car while trying to get away from the explosions.
